Forms for this event must be signed by parents and students, and notarized for all minors in attendance.  Download below and bring them with you on race day.  Adults (18+) can sign form 1155 and do not need to be notarized.

Students will not be allowed to participate or enter the restricted areas without a signed, notarized waiver release.
